Anxiety and fear of childbirth among primigravida women may heighten pain perception, weaken labor self-efficacy, and reduce readiness for normal birth, worsening the birth experience and maternal and infant outcomes. Inadequate stress management and limited family involvement often contribute to psychological distress during pregnancy. This community service program aimed to improve childbirth readiness through the integration of mindfulness practices and family support among third-trimester pregnant women in the working area of Klego I Public Health Center, Boyolali. The program involved 30 primigravida women and their family companions through four stages, including childbirth education, mindfulness training, strengthening family support, and integrated practice sessions. Childbirth readiness was assessed before and after the program using the Modified Normal Childbirth Readiness Questionnaire (KKPN-M), a 20-item instrument covering physical readiness, psychological readiness, childbirth knowledge, family support, and birth planning. Pretest and posttest scores were compared using the Wilcoxon signed-rank test. The findings showed a significant improvement in childbirth readiness after the intervention (p < 0.001). Before the program, 73.3% of participants were categorized as having low readiness, whereas after the intervention, 60.0% were categorized as ready and 40.0% as moderately ready. The average readiness score increased from 32.57 to 52.67. Expressed as the percentage increase over the pretest mean, the greatest improvement was observed in psychological readiness (95.79%), followed by childbirth knowledge (77.03%) and birth planning (64.25%), while family support increased by 29.41% (7.37 to 9.53). The integration of mindfulness and family support effectively enhanced psychological preparedness, knowledge, family involvement, and birth planning among pregnant women. Puskesmas are recommended to integrate this model into routine pregnancy classes and train midwives and health cadres as facilitators, and policymakers to support its replication in primary maternal health programs.
